COVID-19 available for:

  • Phase 1a: Healthcare Personnel
  • Phase 1b: Adults 75+ and Frontline Essential Workers
  • Phase 1c: Molokai Residents Age 65-74, 18-64 with medical conditions, essential workers not in Phase 1a or 1b

Molokai General Hospital and Molokai Community Health Center will be administering the COVID-19 vaccination.

Vaccinations are by Walk-in Basis Only

  • For individuals that received their first dose from Molokai General Hospital
  • Please bring your COVID-19 vaccination card, ID, and Insurance Cards
  • For individuals with Medicare, please bring your red, white, and blue Medicare card with you
  • You may print and fill-out the registration form and questionnaire by clicking on the links below. Filling out the forms is not mandatory prior to arrival at the clinic but having it will reduce your wait time to receive the vaccine.
